Service Description
Please Note: This assessment is not a DUI evaluation. If you are in need of a DUI evaluation, please visit our [DUI Evaluation page] and schedule the appropriate appointment. This assessment is intended to evaluate a client's current or past history with alcohol and/or substance use to determine whether they meet the criteria for substance abuse treatment. It is not designed for individuals who have been charged with a DUI. If you have been charged with a DUI, a formal DUI evaluation is required.
Cancellation Policy
To cancel or reschedule an appointment, please contact us at least 12 hours in advance. Kindly note that the clinician requires a minimum of 2-3 days to complete DUI evaluations thoroughly. Deposits are non-refundable, and a 25% fee will be charged for cancelled sessions and no-shows
